About this programme
An investigation into claims that poor patient care in hospitals is putting thousands of people at risk of serious injury or even death every year. Although the problem was first highlighted during a scandal at Stafford Hospital in 2005, when hundreds died unnecessarily, reporter Declan Lawn finds serious ongoing problems in trusts across the country.
